We’re excited to welcome you to Vesper Music Academy! Please complete this form after your lesson time has been confirmed.

Submit one form for each student enrolling. After we receive it, we’ll finalize the student’s enrollment and send the information needed to access the Parent Portal and set up automatic payments.
